Total Hip Replacement: Relieving Pain and Arthritis in Hips

Hip pain can affect anyone, but when conservative treatments fail, total hip replacement surgery may be the answer to a person's problems. It revolves around the complete replacement of the damaged hip joint by prosthetic parts to relieve pain and restore mobility. It is otherwise called hip arthroplasty. Dr. Akshay Shetty is one of the leading orthopedic surgeons who perform total hip replacement here in HSR Layout, BTM Layout, and Koramangala, making it possible for a patient to again lead a pain-free active life.

What is Total Hip Replacement?

The ball is the head of the femur (thigh bone), while the socket is in the pelvis for the ball-and-socket hip joint. A total hip replacement involves the:

Removing the damaged femoral head and replacing it with a metal stem that's attached to it.

Adding a ceramic or a metal ball on top of the stem acting as a replacement for the damaged femoral head.

Replacing the damaged cartilage of the acetabulum (hip socket) with a metal socket.

Installing a polyethylene or ceramic spacer between the new ball and socket, allowing smooth movement.

Some Common Indications for Total Hip Replacement

Total hip replacement generally is indicated for patients with:

Osteoarthritis: It's a wear-and-tear degeneration at the hip joint accompanying age.

Avascular Necrosis (AVN): Bone collapse occurs due to restriction of blood flow to the femoral head.

Tuberculosis of the Hip: Resulting in erosion of the joint because of an infection.

Childhood Hip Disorders: Perthes disease, slipped capital femoral epiphysis (SCFE), or developmental dysplasia (DDH).

Hip fractures in seniors, whereby a fracture cannot be repaired well.

The Methodology

Patients can be given either spinal anesthetic or major anesthesia for the procedure depending on the healthy condition and choice of the patient. The procedure includes:

Femur Component Fixation: A metallic ball is inserted with stem into femur, the stem could be fixed with either bone cement or a more bio analogous manner called Hydroxyapatite (HA) coating, which promotes natural bone in-growth to have stronger bond.

Acetabulum Component Fixation: The diseased hip socket is replaced with a metallic socket fitted pressed into place with stabilizing hooks or pegs.

Immediate Post-operative Care

After surgery, the patient is motivated to commence his physio rehabilitation the very first day after surgery. The rehabilitation pays attention to:

Passive stretching and active-assisted activities.

Early mobilization; in this way the patient could walk with a walker supporting weight to the leg.

It is planned to discharge per patient from 2 to 4 days; nearly all patients will be able to do day-to-day tasks with minimal help in a few weeks.

Post-operative Precautions

The first three months are usually periods of prevention for patients.

- Sitting on low chairs or the ground.

- Using Indian style toilets.

- Sleeping on the operated hip.

While sleeping, a pillow should be placed between the legs to maintain proper alignment.
